We have just started having a white film on our instruments after sterilization. We are having the water and steam tested. The OR is concerned that the instruments are not sterile. Any suggestions???::

What enzyme and lube's are you using for the instruments? No if there is a film the instruments are not sterile under the film.

It is probably neutralising amines from the steam. Check with whoever provides the steam.

Some questions to ask to track down the source of the film:
*Is the film on handwashed items, as well as mechanically washed items? If so, you can rule out the washers as the source.
*Is the film on items sterilized in your low-temperature units, as well as your steam autoclaves? If so, you can rule out your autoclaves.
*What in your processes has changed recently? Are you using any new detergents, lubricants, enzymatic cleaners? Have you recently cleaned your sterilizers any differently than before? Do you have a new source of purified water?
*Have you just had service on your boilers or steam pipes?

If you are routinely cleaning your sterilizer chamber and the cleaning compound you use is not completely rinsed off you will get a white film. Also, if you are using reusable towels in your sets, check with your laundry--if they have changed processes it can affect the rinsing of soap from the towels. A quick way to test if there is still soap residue in your reusable linen is to get a basin of water and swich a towel in it. It will be apparent if there is any soap residue.
