Does anyone do quality checks on trays to ensure all the instruments are in the tray, working properly and clean?

I'm wondering who does the QC, what part of the process does it occur(prep&pack, after sterilihow often it occurs, what type of education, coaching, discipline occurs if errors are found.

Do you hold the OR's accountable for making sure all the instruments go back into the tray?

I complete a montly QA of a percentage of sets processed and put away. I randomly pull 10-15 sets per month to verify everything was assembled correctly. If you give me your email address I will send you the policy and audit forms that I use.
