Are any of you using the TOSI test? Are you passing it? We are having some problems with it and am wondering what result everyone else is having.

I am having some problems passing it but that is because I do not run an enzamatic detergent in my washers. We are still wroking on finding suitable detergents that do not cause a foaming issue in our washers.

We use the TOSI test and we pass each time. In the beginning we were not getting a clear reading. We checked the washers carefully and discovered that one of our staff took out one of the side plates for a "high tray" on our Getinge 8666 and did not replace it and this resulted in low water pressure coming from the spray arms.
My suggestion is to check your washers carefully and make sure all procedures are being followed by staff.

To be more specific, the TOSI is a method of evaluating the entire cleaning effectiveness of the your automated instrument washer, not just your detergents, but the entire process including the mechanical effectiveness of the washer. If the result is not favorable, you would then need to evaluate the various components that contribute to the effectiveness of the washing process.

You will need to contact the manufacturer of the instruments and the plastic sigmoidoscope tip to find the correct way to sterilize them. They are required to inform you of the sterilization process that it has been validated in.

We passed our TOSI w/o problems but I did hear from another hospital that uses low impingment washers with the same chemicals that we use that couldn't get it to pass.

It is used at my facility and I must agree that at first it did not seem to work. I found that it was the user and not the product. It allows us to tell if the machines are really cleaning the instruments. Once my TOSI was not clean I could determine that the instruments being put through were not clean . I was able to have maintance for the machines done and then the TOSI came out clean. I think it is another tool that allows us to stand behind the product we put out. Remember is must be clean before you can proceed. :D
